The information written in section 1 may not be revised unless both parties agree and sign on it. In order to make a revision, circle the area that needs revising then write the revision neatly beside it. Photocopy the contract twice and write “REVISION” at the top of both. Both parties must sign both photocopies and each party gets one photocopy which they should then staple to their original contract. This is the only way to make a valid revision.

SECTION 2: To be filled out and signed by the artist

After reading the above information provided by the employer, I, Ingrid Glaw quote the price of this job to be  $_______________ .
Once I have received 50% of the payment listed above I shall complete the commission as described in Section 1 by the deadline listed above. Once the commission is complete I will send the employer a low resolution watermarked scan of the image. The employer must then send the remaining 50% of the payment in order to receive the full quality image. If the employer, for some reason is not happy with the end result of the commission, he/she may ask me to make small revisions or he/she may cancel the commission and not pay the remaining 50%. The employer will only be refunded his/her 50% deposit if I fail to follow instruction as written above in Section 1 or miss the deadline.
If the commission is for commercial use, then I give the employer permission to use the image only how they have been written under SECTION 1 #7. The employer may not use the image for another purpose beside those written in SECTION 1 #7 unless he/she gets written permission from the me. If the commission is for personal use then the employer may not use the image for profit making purposes.

SECTION 3

I,_______________the employer, agree to pay Ingrid Glaw 50% of $___________________ inorder for her to start the commision and once I am happy with the commission I shall pay her the other 50%..
I understand that the only way for me get my 50% deposit back is if the artist fails to follow the instruction given in SECTION 1 or she misses the deadline. Canceling the commission means I wont have to pay the remaining 50% but I will not be refunded my 50% deposit. I agree to give the artist credit for their work and not to make false claim, such as claiming the artwork as my own. If the commission is for commercial use I'll only use it for the purposes listed in SECTION 1 #7 unless I get written permission from the artist. I will not alter the image unless the alterations are included in SECTION 1 or if I have received written permission from the artist.  If the commission is for personal use then I agree that I will not resell the image for more then I paid for it, I will not use the image to represent a business or for any other profit making venture. If I ever decide I would like to use the image commercially, I must get written permission from the artist otherwise  it is considered a breach in contract.
